Normal external canal: Skin is pale-pink to tan, smooth, with fine hairs in the outer third. Cerumen is present in variable amounts. The canal is open and patent. The tympanic membrane is pearly gray to light amber, with the cone of light and malleus handle visible.
Acute otitis externa: Erythema and edema of the canal skin are the primary findings. Early OE may show only mild erythema and tenderness with traction on the pinna (the pinna tug test — pain with tragal pressure or pinna pull is pathognomonic for canal rather than middle ear disease). Moderate-to-severe OE shows canal edema that may narrow the lumen substantially, limiting visualization. Serous or purulent discharge, debris, and in fungal cases, white or black filamentous material ("fungal plaques") may be visible.
Limitations of examination with canal edema: When the canal is significantly edematous, TM visualization may be impossible, making it difficult to rule out concurrent AOM. This is clinically important — see the next section.

First-line treatment for uncomplicated bacterial OE: Topical antibiotic/steroid combination drops. The three standard options:
