Summer pool season is here and swimmer's ear cases are about to spike. Here's everything parents need to know to protect their kids — and what to do when prevention doesn't work.
Pool season is finally here. The community pools are open, the kids are jumping in the moment school lets out, and by mid-June, every pediatrician, urgent care, and school nurse will be fielding the same complaint: ear pain in a kid who's been swimming every day. Swimmer's ear — otitis externa — is one of the most predictable seasonal diagnoses in medicine. It happens every summer, it spikes fast, and with a little understanding, most cases are preventable.
The external auditory canal has a natural defense system. The cerumen (earwax) that most people try to remove is actually protective: it's slightly acidic (pH 6–7), which inhibits bacterial growth, and it coats the canal skin in a thin layer that repels moisture. When the ear gets wet repeatedly — pool, ocean, lake, bathtub — water disrupts this protective layer. The acid balance shifts, the skin softens, and bacteria have a foothold.
The dominant culprit is *Pseudomonas aeruginosa*, a gram-negative bacteria that loves warm water. Pool environments are ideal: warm water, trace organic matter, and if the chlorine balance slips even slightly, bacterial counts spike. The canal becomes a petri dish. Once Pseudomonas establishes itself in softened, pH-disrupted canal skin, the infection develops quickly.
The progression of swimmer's ear is recognizable once you know the pattern:
Stage 1 — Itch before pain. The first symptom is usually itching inside the ear canal. This is often dismissed as normal. It's not — it's the early inflammatory response. A kid who keeps sticking a finger or swab in their ear after swimming is in early OE.
Stage 2 — Pain with pinna tug. This is the clinical hallmark. If pressing on the small bump at the front of the ear canal (the tragus) or pulling on the outer ear causes pain, that's swimmer's ear. Middle ear infections (otitis media) don't hurt with external pressure. This test is reliable enough that if pinna tug hurts, swimmer's ear is the diagnosis until proven otherwise.
Stage 3 — Discharge and hearing. As infection progresses: discharge from the canal (clear, then cloudy, then purulent), a feeling of fullness or muffled hearing, and increasingly severe pain that can become constant.
How to tell swimmer's ear from a regular ear infection: The pinna tug test is the key. An ear infection behind the eardrum (otitis media) typically comes with fever and follows a cold or upper respiratory illness. Swimmer's ear comes after water exposure and hurts when you touch the outside of the ear.
The best swimmer's ear is the one that never happens. A few practical steps that actually work:
After every swim: Tilt the head to each side and pull the pinna gently to open the canal. Let gravity drain the water for 30 seconds per side. A hair dryer on the lowest heat setting, held at arm's length, speeds up drying.
Preventive ear drops: A 1:1 mix of white vinegar and rubbing alcohol dropped into each ear after swimming, held for 30 seconds, then drained is a time-tested method that re-acidifies the canal and removes residual moisture. Commercial versions are available at any pharmacy.
Custom ear plugs for frequent swimmers: Children who swim daily — competitive swimmers, kids at swim camp — benefit from fitted ear plugs that keep water out of the canal. Foam plugs are better than nothing but often don't seal well in children. Fitted silicone plugs from an audiologist provide a proper seal.
What NOT to do — cotton swabs: This is the big one. Cotton swabs remove the protective cerumen layer, abrade the canal skin, and often push debris toward the eardrum rather than out. The canal has a natural self-cleaning mechanism (outward migration of dead skin cells). Don't interfere with it.
See a doctor promptly: Pain that has progressed from itch to real discomfort; visible discharge; hearing that feels muffled; fever; or symptoms that aren't improving after 24 hours of keeping the ear dry.
OK to watch for 24 hours: Early itching without significant pain, no discharge, no fever, child is otherwise well. Keep the ear dry (no swimming, wear a cotton ball with petroleum jelly in the canal during bathing), and see if it resolves. If it worsens or doesn't improve, go in.
Never wait with: Very severe pain, swelling or redness spreading beyond the ear canal onto the face, child is immunocompromised, or symptoms in a child under 2.
