The 2026–27 school year is approaching, and school nurses in districts across the country are in planning mode: supply orders, immunization review, health record updates, and — for those with state-mandated screening programs — the annual ear and hearing screening protocol. Hearing screening in schools is one of the highest-yield early detection programs in pediatric health, identifying children with undiagnosed hearing loss that may be affecting their learning before academic difficulties compound.

Otitis media with effusion (OME): Amber or yellow TM, retracted position, absent or diminished light reflex, possible visible fluid level. OME is a common cause of persistent mild conductive hearing loss in school-age children and is frequently asymptomatic — the child has no ear pain, the parents have no complaint, but the otoscope and audiogram together tell the story. This is exactly the population that school screening is designed to find.
TM abnormalities warranting referral: Perforation (visible TM opening with or without discharge); retraction pocket (localized area of TM retraction, particularly in the posterior-superior quadrant, which may indicate early cholesteatoma); tympanostomy tubes in place (note and document — does not require referral unless tube appears blocked, extruded, or surrounded by discharge).
Here is the operational reality that equipment marketing rarely addresses: a school nurse conducting a mandated hearing screening week may examine 50 to 200+ students over five to seven days. At 2–3 minutes per student including the otoscope exam, that is continuous patient contact for hours at a time.
Between-student disinfection is a clinical requirement. The otoscope makes direct contact with the speculum (disposable) but the handle, head, and battery compartment are handled by the same gloved hands that touched the previous student's auricle. Hospital-grade disinfectant wipe-down of the instrument between students is the appropriate infection control standard.
Disposable specula are required. No reuse between students under any circumstances.

Refer to physician (not audiologist) as first step: - Signs of active infection: erythematous canal, discharge, pinna tenderness - Suspected foreign body - Any child who appears ill
Document and communicate: Referral letters should include the specific otoscope findings and audiometric results that prompted referral, in language parents can understand. "Your child has fluid behind the eardrum in both ears that may be causing mild hearing difficulty" is actionable for a parent. "Failed hearing screening" is not.
