Ear infections are culturally coded as a childhood problem. The reality is that acute otitis media (AOM) affects adults too.
Why Adults Get Ear Infections
The anatomy of the eustachian tube changes substantially between childhood and adulthood. In young children, the tube is short, horizontal, and poorly compliant. By adulthood, the tube is longer, angled more steeply downward, and generally more efficient at drainage. This anatomical maturation is why AOM incidence drops sharply after age five.
Upper respiratory infection remains the most common trigger. Viral URIs cause mucosal edema that obstructs the eustachian tube orifice, impairs mucociliary clearance, and creates the conditions for bacterial superinfection.
Eustachian tube dysfunction from allergic rhinitis is a major and underappreciated cause. Seasonal allergy sufferers with poorly controlled rhinitis experience chronic low-grade tubal obstruction that predisposes to recurrent OME and occasional AOM.
How Adult AOM Presents
Pain is the cardinal symptom. Adult otalgia from AOM is typically described as a deep, throbbing pressure that worsens when lying down. Adults frequently present believing they have sinus disease or a dental problem.
Referred pain is a significant diagnostic confounder. The ear receives sensory innervation from multiple cranial nerves (V, VII, IX, X) and the C2–C3 cervical nerves. In adults, the differential diagnosis for otalgia is wider than in children, and AOM is less immediately on the list.

The key diagnostic criterion is TM bulging, not erythema. A red tympanic membrane without bulging can be produced by fever or forceful nose-blowing. Bulging — the membrane displaced anteriorly or posteriorly under middle ear pressure — is the finding that confirms active infection.
Treatment Decisions in Adults
The watchful waiting option. For adult patients with mild unilateral AOM, no systemic symptoms, and no immunocompromise, a 48–72 hour observation period with analgesics and a safety-net prescription is reasonable.
When to treat immediately. Bilateral AOM, severe otalgia lasting >48 hours, fever ≥39°C, or immunocompromise warrant immediate antibiotic therapy. First-line: amoxicillin 500 mg three times daily for 5–7 days.
Address the underlying cause. Persistent unilateral middle ear fluid in an adult without a clear cause should prompt referral for nasopharyngoscopy to rule out a structural cause.
